What are the key transesophageal echocardiography (TEE) views and indications for TEE in suspected endocarditis?

Mid-esophageal 4- and 2-chamber, long-axis; transgastric views

In suspected endocarditis, you want imaging that gives the clearest, most comprehensive look at the valves and the surrounding structures where infection tends to hide. The best combination of TEE views for this is mid-esophageal four-chamber and two-chamber, plus the mid-esophageal long-axis, with supplemental transgastric views.

The mid-esophageal four-chamber view is ideal for seeing the mitral and tricuspid valves and the left atrium, letting you detect vegetations that may cling to the leaflets or annulus. The two-chamber view complements this by focusing on the anterior and posterior aspects of the mitral valve and its relationship to the left atrium and LV. The long-axis view from the mid-esophagus adds a different plane that prominently shows the aortic valve and the LVOT, which is crucial for identifying involvement of the aortic valve, root, or adjacent structures such as an abscess or perforation that can accompany endocarditis.

Transgastric views, obtained from the stomach, provide a close-up perspective of the mitral valve apparatus and annulus from the LV perspective and can better delineate vegetations or infection around the mitral valve, as well as offer valuable assessment of LV function and the aortic valve from below. This combination yields the most sensitive and reliable assessment for endocarditis, including vegetations, abscesses, and complications, and helps guide surgical planning.

Indications for using TEE in suspected endocarditis include high clinical suspicion with persistent bacteremia, a new or changing heart murmur, prosthetic valve infection, or when transthoracic echocardiography is nondiagnostic or limited. You want the targeted esophageal views above because they maximize visualization of the valves and adjacent structures where endocarditic pathology typically resides.
